Getting started
Do I need a doctor's referral?
No. Georgia is a direct-access state, which means you can see a physical therapist without a referral. The one exception is dry needling — Georgia state law requires a physician referral for that specific service. Once you have one, it's good for one year.
How quickly can I be seen?
There is no waitlist. Book online and you'll see the next available openings in real time. If you need help finding a time, call or text and Dr. Nicole will book one for you.
What conditions do you treat?
Pelvic floor conditions for women across every life stage — pregnancy, postpartum, prolapse, incontinence, pelvic pain, painful intercourse, constipation, and more. Do you treat men?
Yes — for orthopedic, vestibular, dry needling, and wellness services. Pelvic floor physical therapy at this practice is for women only.
Your visit
How long is a typical visit?
One full hour, every visit. Same length for an evaluation as for a follow-up. Often the first visit runs slightly longer if scheduling allows.
What should I wear?
Comfortable clothes you can move and stretch in. Stretchy pants or shorts work well. You don't need to bring anything special.
Can I bring my baby or kids?
Yes. The treatment room has space for a stroller. Many postpartum patients bring their babies, and Dr. Nicole — a three-time mom herself — is genuinely comfortable with kids in the room.
Can my partner come with me?
Absolutely. You're welcome to bring your partner, a parent, or a friend for any visit — especially the first one.
Do I have to do an internal pelvic exam?
Never. Internal exams are useful diagnostic tools but they are not required at any visit. Many patients stay external for the entire course of care. You stay in control of every decision.
How many visits will I need?
Most patients see real improvement in three to four visits. Some chronic or complex conditions take six to eight. Plans are built around what your body actually needs — not what insurance allows.
Pricing & insurance
How much is a visit?
$125 for a one-hour session (same rate for evaluations and follow-ups). $60 for a 30-minute wellness visit (dry needling, cupping, or manual therapy on its own). Six-visit packages save 10%.
Do you take insurance?
Bremen Pelvic Health is a cash-pay (out-of-network) practice. Insurance is not billed directly. Many patients are reimbursed by their insurance using a superbill, which is provided after every visit at no extra cost.
Do you accept HSA or FSA?
Yes — HSA and FSA cards are accepted as payment, just like a debit card. For most patients with high-deductible plans, paying with HSA makes the cost comparable to in-network care.
How do superbills work?
After every visit, you'll receive an itemized invoice (the superbill) with the diagnosis and treatment codes your insurance needs. You submit it to your insurance through their member portal or by mail, and they reimburse you directly for whatever your out-of-network plan covers. Some patients use a service like Reimbursify to file for them.
What forms of payment do you accept?
Venmo, Cash App, Zelle, and cash (preferred — no processing fees). All major credit cards. HSA and FSA cards.
About Dr. Nicole
What are her credentials?
Dr. Nicole McDonald is a Doctor of Physical Therapy (DPT) with 12 years of clinical experience. She is a Board-Certified Orthopedic Specialist (OCS) through the APTA, completed her IAMT Pelvic Health Certification in 2024, and is certified in dry needling (Cert-DN), ASTYM, and manual therapy.
Did she train in pelvic floor PT specifically?
Yes. Her pelvic health certification is a four-course program through the Institute of Advanced Musculoskeletal Treatments (IAMT) — the same residency program where she completed her orthopedic specialty. The combination of board-certified ortho plus pelvic certification is unusual and is the foundation of how she approaches every case.
Is she taking new patients?
Yes. Bremen Pelvic Health currently has no waitlist. Book online to see the next available openings.
What about her maternity leave?
Dr. Nicole is expecting her third child in late October 2026. She plans to work up to her due date and return in approximately 12 weeks. New patient bookings will pause briefly during her leave — the website and online booking page will display the most current return date.
Logistics
Where is the clinic?
1135 Pacific Ave, Bremen, GA 30110 — inside Ladies Way Fitness. The plaza is across from Blue Devil Park, near the ball fields.
Do you offer telehealth or in-home visits?
No. All care happens in person at the Bremen clinic. Patients traveling from out of town are welcome to combine multiple appointments into a single trip.
What's the cancellation policy?
We ask for 24 hours' notice when possible so the slot can go to another patient. There is no fee for late cancellation — but please give as much notice as you can.
